We are seeking a compassionate and experienced Wound Care Nurse Practitioner (NP) to join our dynamic team at Southern NH Health. In this role, you will play a vital role in providing comprehensive wound care services to a diverse patient population in an outpatient setting.
Responsibilities:
- Conduct comprehensive wound assessments, including evaluating cause, stage, and underlying conditions.
- Develop and implement individualized treatment plans using the latest evidence-based practices and technologies.
- 36 patient contact/4 administrative hours per week, no nights or weekends
- Order and interpret diagnostic tests, such as cultures and biopsies.
- Outpatient with opportunity for inpatient consults, perform debridement, dressing changes, and other wound care procedures
- Prescribe medications, including antibiotics and pain management medications.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient care.
- Educate patients and families on wound care, prevention strategies, and self-management techniques.
- Document patient encounters and progress notes accurately and thoroughly.
- Stay up-to-date on the latest advancements in wound care through continuing education and professional development.
Qualifications:
- Master’s degree in nursing and Wound Care certified
- Wound care preferred
- Strong clinical skills, including assessment, diagnosis, and treatment planning.
- Excellent communication, collaboration, and interpersonal skills.
- Proficiency with EPIC EMR preferred
- The ability to work independently and as part of a team.
